The Florida Department of Financial Services previously had special paperwork requirements for annuity business in Florida. Forms DFS-H1-1980 and DFS-H1-1981 were required, however they have been discontinued effective 01/01/2024. ESI has removed these forms from Docupace since they are no longer required.
